School Physicals

I.Each sI. Each student enrolled in the District shall have a physical examination administered by the school physician or family physician in Pre-K, K, Grade 2, 4, 7, 10, or as a new enrollee, in accordance with the provisions of the New York State Education Law.If a physical report from the private physician is not received by the school nurse by the time of the school physicals, the child will automatically be included with the school physicals.​II. 

II. Upon enrollment in the District, all pupils shall be given an eye test for color perception, distance acuity, and near vision within six (6) months of admission to school, in accordance with section 905 of the New York State Education Law.

III. A. Medical examinations are to include:
    1. Eye tests must be conducted at least once in a school year.2.  

    2. Hearing tests must be conducted at least once in a school year in grades Pre-K through 7, grade 10, and at other times deemed necessary.  
  
    3. Pupils in grades 5-9 must be examined for scoliosis once each year.

B. Health examinations may be required by school authorities at any time to promote the educational interests of the child.

IV. The District shall advise, in writing, the parent(s) or guardian(s) of each child in whom the health services provided by the District indicate a defect or other condition which may require professional attention.

V. The District shall ensure that the results of health examinations are recorded on approved forms which shall be kept on file in the District.  The health records of individual children shall be kept confidential, except as such records may be necessary for the use of approved school personnel and, with the consent of the parents or guardians, for the use of appropriate health personnel of cooperating agencies.
